Sign up for our screening event!

Lively Therapy Services is hosting a screening event for physical therapy services. During the screening process, your child will be observed completing several developmentally determinate activities in which our physical therapy department will determine if a further evaluation for services may be beneficial. Please sign up below and a member of the staff will contact you with your scheduled time. Time slots are limited and are assigned on a first come, first served basis. There is no charge to have a screen completed. You will be required to complete minimal paperwork allowing us to screen your child and contact you about the results.
